summary
Ohio EPA conducted a reconnaissance inspection of the Lima WWTP on July 25, 2017, following a lightning strike and power outage on July 22, 2017. The strike damaged the main switchgear, causing loss of pumping capacity, overflow of final clarifiers and aeration tanks, and flooding of the plant with substantial damage. At the time of the inspection, power had been partially restored and temporary generators were in use, with final effluent to the Ottawa River appearing clear and odor-free. Compliance data for the period 3/1/2017–7/1/2017 showed zero effluent limit violations but 10 code events and 68 reported SSO events.

⚠ Gaps in the record
- TWIN CAPTURE — not the canonical reading. The portal serves this 2017-07-25 reconnaissance inspection at two docids; edoc-1851474 is the same letter and is canonical for FIDELITY (a scan is read by the vision path alone, with nothing to check itself against). Declared in data/site/document-versions.yaml as a `v2` cluster — the two files are NOT byte-identical.
- Document read from page images; text layer appears present and legible throughout all four pages.
- The NPDES permit ID on the letter Re: block reads '2PE00000' (permit_no) and the form shows NPDES # '2PE00000' with Ohio permit OH0026069; both are recorded accordingly.
- Permit expiration date (2/28/2015) predates the inspection date (7/25/2017), which may indicate a permit renewal was pending — noted but not recorded as a finding per instructions.
- Exit time on form reads '12:00' with no AM/PM indicator; recorded exactly as printed.
