summary
On June 30, 2021, Ohio EPA conducted a Pretreatment Compliance Inspection (PCI) via comprehensive desktop review of the City of Lima's approved pretreatment program. Key findings include a pending (unapproved) local limit study submitted in February 2019, a discrepancy between the eight industrial users under permits and the 12 significant industrial users listed in the annual report, and an ongoing investigation into surfactants discharged to the WWTP. Three advisory recommendations were issued, including updating the sewer use ordinance once the local limit study is approved, conducting an industrial user inventory, and identifying non-significant categorical users upstream of combined sewer overflows.

⚠ Gaps in the record
- The significant_noncompliance field is left null because no dedicated 'Sig. Non-Compliance' checkbox result was clearly readable on the attached inspection form image; it should be verified against the physical form.
- The attached checklist pages were partially read via OCR; numeric and coded fields were cross-checked against the document images where possible.
