summary
This letter is the City of Lima's response to Ohio EPA's Compliance Review Report dated July 20, 2016, concerning the City of Lima WWTP (NPDES Permit No. 2PE00000). The compliance issue identified was a Total Residual Chlorine reading of 0.30 mg/l at Station 001 against a limit of 0.02 mg/l, caused by inadequate chlorine feed rate adjustment following a rain event. As a corrective measure, training was provided to all plant operators on responding to flow changes.

⚠ Gaps in the record
  • This document is a facility response letter to a Compliance Review Report, not the inspection/compliance report itself — no attached EPA inspection form is present, so many fields (inspection_date, entry_time, exit_time, inspectors, significant_noncompliance, type_code, units_in_service) cannot be populated.
  • The permit number in the Re: block reads '2PE00000*MD' — the '*MD' suffix may be a modifier or OCR artifact; base permit number recorded as 2PE00000.
  • No NPDES federal ID (OHxxxxxxx) is printed in this document.
  • County (Allen) is inferred from Lima, Ohio — not explicitly stated in the document.
  • Document was read from a page image with an unreliable OCR text layer; values verified against the image.
